Requesting records:

To request your health care records, please contact your clinician directly through the portal or by contacting the office. Please note that a nominal fee applies. You will receive a Release of Information form to specify the recipient of your records. Once you have completed the form and paid the required fee, your records will be sent to the designated entity.

You can file a consumer complaint with either agency below.
